All business license applications submitted by any person or
entity wishing to provide basic life support ambulance ("BLS ambulance")
and/or critical care transport ("CCT") services within the city shall
submit the following with the business license application:
A.
The
person or entity's county-issued BLS ambulance and/or critical care
transport permit; and
B.
The
person or entity's application for BLS ambulance and/or critical care
transport permit which was submitted to the county, including all
supporting materials and documentation.
