Applications for a wastewater discharge permit shall be accompanied by a discharge report. The report shall include, but not be limited to, nature of process, volume, rates of flow, mass emission rate of BOD, suspended solids, total dissolved solids, sodium and chloride, production quantities, hours of operation, number and classification of employees, or other information which relates to the generation of waste including wastewater constituents and characteristics in the wastewater discharge as determined by sampling and testing methods approved by the Director of Utilities. Such reports must include chemical constituents and quantity of liquid or gaseous materials stores on site even though they may not be discharged. In addition to discharge reports, the City may require information in the form of wastewater discharge permit applications and self-monitoring reports.
