All Class III and Class IV users proposing to connect to or discharge into a community sewer must obtain a wastewater discharge permit before doing so. Class III or Class IV users connected to or discharging into a community sewer must obtain a wastewater discharge permit within one year after the effective date of the ordinance from which this section derives.

Applications for a wastewater discharge permit shall be accompanied by a discharge report. The report shall include, but not be limited to, nature of process, volume, rates of flow, mass emission rate of BOD, suspended solids, total dissolved solids, sodium and chloride, production quantities, hours of operation, number and classification of employees, or other information which relates to the generation of waste including wastewater constituents and characteristics in the wastewater discharge as determined by sampling and testing methods approved by the Director of Utilities. Such reports must include chemical constituents and quantity of liquid or gaseous materials stores on site even though they may not be discharged. In addition to discharge reports, the City may require information in the form of wastewater discharge permit applications and self-monitoring reports.

The following information may be required to be submitted in the application for a wastewater discharge permit:
(a) 
Name, address and SIC number of applicant;
(b) 
Volume of wastewater to be discharged;
(c) 
Wastewater constituents and characteristics as required by the Director of Utilities that may include but not be limited to those mentioned in Article 4 as determined within the past 30 days by a laboratory approved by the Director of Utilities;
(d) 
Time and duration of discharge;
(e) 
Average and 15 minute peak wastewater flow rates, including daily, monthly and seasonal variations, if any;
(f) 
Site plans, floor plans, mechanical and plumbing plans and details to show all sewers and appurtenances by size, location and elevation;
(g) 
Description of activities, facilities and plant process on the premises including all materials, processes and types of materials which are or could be discharged;
(h) 
Each product produced by type, amount and rate or production;
(i) 
Number and type of employees and hours of work;
(j) 
Any other information as may be deemed by the Director of Utilities to be necessary to evaluate the permit application.

Wastewater discharge permits may contain the following terms and conditions:
(a) 
The average and maximum wastewater constituents and characteristics permitted;
(b) 
Limits on rate and time of discharge or requirements for flow regulations and equalizations;
(c) 
Requirements for user installation of monitoring facilities;
(d) 
Pretreatment facility requirements;
(e) 
Specifications for self-monitoring programs which may include, but not be limited to, sampling locations, frequency and method of sampling, number, types and standards for tests and reporting schedule;
(f) 
Requirements for submission of technical reports or discharge reports;
(g) 
Requirements for maintaining and retaining, as specified by the City but in no case less than three years, plant records relating to wastewater discharge, and affording City access thereto;
(h) 
Mean and maximum mass emission rates, or other appropriate limits when incompatible pollutants are proposed or present in the user's wastewater discharge;
(i) 
Other conditions imposed by the City to ensure compliance with this chapter.

Wastewater discharge permits shall be issued for a specified time period for not less than 30 days and not to exceed five years or may be stipulated to expire on a certain date. If the user does not notify the City 30 days prior to the expiration of the permit, the permit shall be deemed to have expired, and the user must make new application. In any event the permit must be renewed each five year period. The terms and conditions of the permit may be subject to modification and change by the City during the life of the permit as limitations or requirements modify and change. The user shall be informed of any proposed changes in his permit at least 30 days prior to the effective date of change. Any changes or new conditions in the permit shall include a reasonable time schedule for compliance.

Wastewater discharge permits are issued to a specific user for a specific operation; wastewater discharge permits shall not be reassigned, transferred or sold.

Any user who violates the conditions of the permit, this chapter or applicable State and federal regulations, or any of the following is subject to having the permit revoked:
(a) 
Failure of a user to report the wastewater constituents and characteristics of the user's discharge;
(b) 
Failure of the user to report and secure advance approval or conditional approval of significant changes in operations, or wastewater constituents and characteristics;
(c) 
Refusal of reasonable access to the user's premises, or other premises controlled by the user, for the purpose of inspection and/or monitoring;
(d) 
Violation of special conditions of the permit.
