An application, in writing, must be filed with the superintendent of streets, and upon receipt of such application the superintendent of streets shall make an inspection of the surface to be broken, measure the extent of the aperture and set forth such measurements and location on the permit, which permit shall require the applicant to deposit with the city clerk an amount equal to one dollar per square foot of the aperture, or the execution of a corporate surety bond in a form approved by the city attorney in an equal amount to the city conditioned for the faithful performance of the requirements set forth in this chapter, and then such permit shall be issued by the superintendent of streets or his deputy.
