In addition to the information required by Chapter 1 of this Part, the application shall include all of the following information:
(a)
The dates for which the license is desired.
(b)
The location at which the circus or carnival will be conducted.
(c)
A description of each food distribution booth that will be operated on the business premises.
(d)
The type and number of public drinking facilities to be used.
(e)
The type, number and location of toilet facilities that will be available for use by each sex.
(f)
A description of the provisions to be made for general sanitation of the grounds, booths and structures and for the disposal of garbage.
