In addition to the Conditional Use Permit submittal requirements, an applicant for a large residential care facility shall submit the following information with the application materials for the Conditional Use Permit:
(a) 
Information regarding the State license that is required to operate the large residential care facility, including the license type and information regarding the State department approving the license.
(b) 
The maximum number of residents who may be served by the facility at any one time.
(c) 
The number of support staff who will be employed at the facility, including the maximum number of staff who may be present at the facility at any time.
(d) 
The estimated or average term of a resident’s stay.
(e) 
Information detailing the support services to be provided on-site.
(f) 
Rules of conduct for residents, if any, and/or a management plan.
(g) 
A list of the addresses of all similar facilities that the applicant operated in California within the past five years. The applicant shall certify under penalty of perjury that none of the facilities has been found by State or local authorities to be operating in violation of State or local law.

A list of other similar facilities within the area, and a statement demonstrating that the facility meets any separation requirements that may be required by State law.

(a) 
Off-street parking shall be provided in compliance with the requirements of Section 87.0615 of this Code.
(b) 
The total number of occupants that occupy the facility shall be compliant with the occupancy requirements of the facility.
(c) 
The facility shall meet all requirements for the approval of such use imposed by the City’s Building Code and Fire Code.
(d) 
The development of a large residential care facility shall comply with all requirements of the subject zone and be compatible in design with the surrounding neighborhood.

(a) 
The facility shall be operated in compliance with applicable State, Federal, and local law and shall maintain all necessary State licenses required to provide services.
(b) 
The operator shall comply with the City’s business license provisions.
(c) 
The facility shall not provide any services other than those the facility is authorized and licensed to provide by the State of California.
(d) 
Large residential care facilities shall operate in conformance with the rules of conduct and/or management plan that was submitted as part of the application for a Conditional Use Permit. Violations of those conditions and/or the conditions of approval may result in a revocation of the Conditional Use Permit by the Planning Commission.
