A.
No curb, curb cut, gutter, sidewalk or driveway shall be constructed or reconstructed in any public street in the Village of Great Neck unless application to perform said work shall have been made in writing to the Village Clerk on blank forms provided by the Village. The application shall specify the location and character of the proposed improvements, and no work shall be done until a written permit, signed by the Village Clerk, has been issued on said application, the grade has been approved by the Village Superintendent of Public Works and, in the case of curb cuts, approval has been given as more particularly provided in § 485-27 hereof.

B.
Such application, numbered consecutively, shall be executed in triplicate, one copy to be retained by the applicant, one copy by the Village Clerk and one copy to be forwarded to the Superintendent. The copy retained by the applicant shall be in the possession of the foreman or contractor on the site of the work and must be shown to the proper authorities when requested.

C.
Each application for a curb cut shall be accompanied by a nonrefundable permit fee and a deposit to assure the proper replacement of all damaged curbs, both in such amounts as shall be prescribed from time to time by the Board of Trustees.

D.
Each application for a permit hereunder shall also be accompanied by a certificate of insurance, in form approved by Village Clerk and issued by an insurance company authorized to do business within the state, running to the Village, guaranteeing that the applicant has provided public liability coverage of not less than $100,000/$300,000 and property damage insurance in the amount of not less than $50,000/$100,000 to save the Village harmless from all claims, actions and proceedings brought by any person for injury to person or property resulting from or occasioned by any fault or default by the Village or its employees or agents or by the person to whom the permit is issued or anyone acting thereunder. The policy shall also protect the Village in such amount as shall be prescribed from time to time by the Board of Trustees against any damage caused to any public street or other public property by reason of the work done pursuant to any permit issued hereunder.

All work is to be done in conformity with this article.
