A. 
Base rate. The base rate is $800 per transport, which shall include the dispatch of EMTs; services of basic life support; and emergency ambulance transportation to local healthcare facilities.
B. 
Mileage charge. A mileage charge of $30 per mile shall be added to the base rate.
C. 
Other fees:
(1) 
Oxygen administration: $100.
(2) 
Disposable cervical collar: $35.
(3) 
AED administration: $95.
(4) 
EpiPen®: $200.

Nonemergency (scheduled) stretcher transport: $500.
(6) 
Refusal fee (treatment without transport): $150.
(7) 
Extra attendant/bariatric: $125.
