No person, firm or corporation shall dig, open or excavate any street, highway, alley, roadway or other place of public travel within the Township of Monroe for any purpose whatsoever, including but not limited to the installation of water, gas or sewer pipes or electrical or cable television installations, unless a permit is first obtained from the Superintendent of Public Works subject to the conditions hereinafter set forth:

No street opening permit shall be issued other than for a utility repair emergency for a period of five years from the date of completion of the repaving or reconstruction of said road.
C.
Prior to issuing any road opening permits, the Superintendent of the Division of Streets or his/her designee must approve who establishes the amount of cash for performance bond or the like to ensure the cost necessary for the proper closing.
D.
All road or street openings should be with the expressed written approval from the Superintendent of the Division of Streets of the Township of Monroe or his/her designee.
E.
Prior to the issuing of the road or street opening permit, said request shall be submitted to the Superintendent of the Division of Streets for his review, approval and inspection.
F.
In the event that the street opening permit is requested by any township, county or state agency, the permit fee shall be waived. However, prior to the issuing of the road or street opening permit, said request shall be submitted to the Engineer of the Township of Monroe or Superintendent of the Division of Streets for his review, approval and inspection.
G.
Upon requesting the Township of Monroe to issue a road opening permit, the applicant agrees to hold harmless the Township of Monroe, its officers, agents and servants from and against any loss, injury or damage resulting from any negligence or the fault of the applicant in the case of work covered by the permit. The applicant shall file with the Township of Monroe a certificate of insurance with limits of $1,000,000 for injuries and $1,000,000 for property damage, which insurance shall name the Township of Monroe, its officers, agents and servants as additional insureds.
