Fees shall be as follows:
A. Vital statistics.

(1) Certified copies or certifications of all vital records:

(a)
Birth and marriage records: $20 each certified copy or certification.
(b)
Death records: first 10 certified copies or certifications of
the same record on the same request: $10 per copy.
(c)
Every copy over 10 copies of the same record on the same request:
$5.
(2) Marriage license: $3 in addition to state fee.
(3) Burial permits: $5 per copy in addition to state fee.

(4) Corrections to vital records: $25 for each correction
form filed.

(5) Domestic Partnership Registration: $3 in addition
to state fee.

B. Human Services Division. The following sliding scale
is used for services provided as assistance for programs for which
a client may qualify due to financial hardship and will be repealed
as of January 1, 2023.

(1) Income $40,000 and under: $74.
(2) Income $30,000 and under: $45.
(3) Income $20,000 and under: $26.
(4) Income $40,001 and over: $100.
(5) Insurance or third-party pay: $100.
(6) Family income under $10,000: set your own fee.
(7) Individual income under $6,000; set your own fee.
C. Nursing Division.

(1) PSA: third-party lab fee plus $5 administration fee.
(3) Hemoglobin: $10 for participants not registered in child health conference.
(4) Colorectal screening: $15.
(5) SMAC: third-party lab fee plus $5 administration fee.
(6) Blood type: third-party lab fee plus $5 administration fee.
(7) Child health conference: $10 one-time registration fee; $10 administration/clinic
fee per child per session.
(8) Flu shot: no cost.

(9) Adult vaccination clinic: $15 one-time registration fee; $10 administration/clinic.

Pneumonia shot: equivalent to current Medicare reimbursement
rate for same.

Lipid panel profile screening: $20.

D. Environmental Division.

(1) Food handler course: $40 per student.
(2) Boiler inspection fee: $7 per dwelling unit.
(3) Boiler inspection late fee (late fee applies to reregistration
of heating units after November 30 of the current heating year): $10
per dwelling unit.
(4) Plan review fee: $175 per plan.
(5) Plan
alteration review fee: $125 per plan.
E. Returned check fee. There shall be a $20 fee assessed
for any check returned for insufficient funds or any other reason
deemed necessary by the financial institution.
F. Any fees paid shall be nonrefundable.
