the Murrysville Chief Administrator on or before the controlling state

is delayed for any reason, an appointee shall promptly file same before
commencing their official duties.
An appointee who fails to submit a volunteer's application for
appointment form duly completed and executed for a position on an
agency, board, commission or committee shall be subject to removal
or recision of their appointment, upon written notice thereof, by
the Municipality of Murrysville Chief Administrator. The removal action
shall be effective 30 days after the mail date of the Chief Administrator's
written notice to said appointee of and the appointee's failure to
correct the said lapse in submitting a required form. The removal
action may be appealed to the Council of the Municipality of Murrysville
within 30 days from the effective date of said removal. An appeal
