[Adopted 1-8-1980 by Ord. No. 1712 (Ch. 49, Art. XVII, of the 1979 Code); amended in its entirety at time of adoption of Code (see Ch. 1, General Provisions, Art. I)]
The position of Registrar and Secretary and Treasurer for the Health Department and Health Advisory Board of the City of Garfield be and is hereby established.
The duties of the Registrar shall be as follows:
A. 
To strictly and thoroughly enforce the law relative to the disposal of dead bodies and the registration of vital records in the registration district.
B. 
To supply forms of certificates to such persons as require same.
C. 
To supply to every physician, midwife and funeral director a copy of the law relative to the registration of vital records and the disposal of dead bodies, together with such rules and regulations as may be prepared by the State Registrar relative to their enforcement.
D. 
Execute all certificates of birth, marriage and death and insert the date of filing on each certificate.
E. 
Examine each certificate of birth, marriage or death when presented for recording in order to ascertain whether or not it has been made in accordance with law and the instructions of the State Registrar, and, if such certificate is incomplete and unsatisfactory, the Registrar shall have the same corrected.
F. 
At the expense of the municipality, make a complete and accurate copy of each birth, marriage and death certificate registered by the Registrar on a form or in a manner prescribed by the State Registrar, to be preserved in the office of the Registrar as the local record.
G. 
On the 10th day of each month or sooner if requested by the department, transmit to the State Registrar all original birth, marriage and death certificates received by the Registrar for the preceding month, and the Registrar shall, on or before the 10th day of the following month, report that fact to the State Registrar on a card provided for such purpose.
H. 
Make an immediate report to the State Registrar of any violation of N.J.S.A. 26:6-1 et seq., 26:8-1 et seq. or 37:1-1 et seq., coming to the knowledge of the Registrar.
I. 
In the case of any birth in the registration district to parents who are residents of another registration district or of the marriage in the registration district of any couple who obtained the marriage license in another registration district or of the death in the registration district of any person who at the time of such death was a resident of another registration district, notify the Registrar of the other registration district, within five days of such birth, marriage or death, on forms prescribed by the State Registrar. All entries relating to cause of death on the original certificate must be entered on the death form sent to the Registrar of the other registration district.
J. 
Prepare a monthly report, including a financial statement to the Health Department or Health Advisory Board members, the age, date of death, name and address and cause of death of persons over 21 who have died.
K. 
File with the Commissioner of Registration once a month the age, date of death and name and address of all persons over the age of 21 years who have died during the previous month.
A. 
The duties of the Secretary shall be as follows:
(1) 
To keep all correspondence and convey same to the Health Department or Health Advisory Board members at the monthly meeting.
(2) 
To take minutes at each meeting.
(3) 
To read minutes from the previous meeting.
(4) 
To send out any correspondence from the Health Department or Health Advisory Board members.
(5) 
To mail out applications at the end of the year to all food handlers for licenses.
(6) 
To record all applications.
(7) 
To send out current licenses.
(8) 
To keep a ledger of all money collected.
(9) 
To follow up on applications not returned.
B. 
Typing, stenography and office experience shall be required.
The duties of the Treasurer shall be as follows:
A. 
To deposit all money collected from licenses into the bank every 48 hours.
B. 
To make a check payable to the City Treasurer once a month.
C. 
To keep a ledger of all money deposited and forwarded to the City Treasurer.
The salary for the positions of Registrar, Secretary and Treasurer shall be fixed annually in the Annual Salary Ordinance. [1]
