A. 
A permit from the Village Clerk shall be required to extend a driveway or walkway across the berm to the paved road.
B. 
The application for a curb cut shall include the following on a plot plan of the property in question:
(1) 
Property lines.
(2) 
Location of existing curb cut (if any) and driveway.
(3) 
Location and dimensions of proposed curb cut.
(4) 
Location of anything in the proposed curb cut area or within 20 feet thereof, i.e., tree, fire hydrant, etc.
(5) 
Location of trench drain and dry well as required by this chapter.
(6) 
Any other information requested by the Village Clerk.
C. 
Any permit granted shall be upon the condition that such change shall be made according to the approved plans and the requirements of this chapter.
D. 
Curb cuts are limited to one per property and only upon a property with a primary dwelling thereon.
E. 
Curb cuts are limited to a maximum width of 18 feet while within the berm area.
F. 
All portions of a driveway, driveway apron or curb cut located on the berm shall lie within the projected lot lines extended to the curb.
G. 
Any project requiring a new or revised curb cut shall include a trench drain on the applicant’s property which shall cross the full width of the driveway. Said drain must be connected to a dry well capable of containing a rainfall of two inches within a twenty-four-hour period for the surface area to be drained.
H. 
If permit conditions are not complied with, said permit shall be revoked and the berm restored at the expense of the applicant
