(A) 
Definitions. For the purposes of this section:
SUBSURFACE SEWAGE DISPOSAL SYSTEM
Any septic tank followed by leaching trenches, pits, beds or galleries.
WATER SUPPLY WELL
Any artificial excavation, constructed by any method, for the purposes of getting water for drinking or other domestic use.
(B) 
Subsurface sewage disposal system permit:
(1) 
Required. Pursuant to the State Health Code, Section 19-13-B20A through Section 19-13-B20S, a person intending to construct, alter, repair or rebuild a subsurface sewage disposal system shall first obtain a permit from the Director of Health.
(2) 
Fees. The fees for such a permit shall be as follows:
(a) 
New septic system application fee: $50.
(b) 
New septic system permit fee: $50.
(c) 
Application for repair, replacement or addition to septic system: $50.
(d) 
Permit fee for repair, replacement or addition to septic system: $30.
(e) 
Application and permit fees for subdivision soil suitability investigation and testing, per lot: $50.
(C) 
Water supply well permit:
(1) 
Required. Pursuant to the State Health Code, Section 19-13-B51A through Section 19-13-B51M, a person intending to construct a water supply well shall first obtain a permit from the Director of Health.
(2) 
Fee. The fee for such a permit shall be $30.
