A certified survey map prepared by a registered land surveyor
shall be required for all minor subdivisions that are within the Village
boundaries or within the Village's area of extraterritorial review.
It shall comply in all respects with the requirements of § 236.34,
Wis. Stats. The map shall show correctly on its face, in addition
to the information required by § 236.34, Wis. Stats., the
following:
A. Date, scale and North arrow.
B. Name and address of the owner, subdivider and the surveyor, including
his registration number.
C. Size of the parcels being created in square feet.
D. Floodplain, shoreland or wetland boundaries.
E. All existing structures, paved areas, watercourses, drainage ditches,
easements and other similar significant features pertinent to proper
land division.
F. Setbacks or building lines from public rights-of-way or shorelines
as provided in the Village Zoning Ordinance.
G. All land reserved for dedication or future acquisition, including
dimensions and areas.
H. Location and names of any adjoining streets, highways, subdivision,
parks, cemeteries, public lands and watercourses. The owners of record
of any unplatted lands shall also be shown.
I. Exact street width along the line of any obliquely intersecting street.
J. Notations or any restrictions required by the Village Zoning Administrator
or Village Plan Commission or other approving or objecting authority
relative to access control along any public ways within or adjacent
to the proposed certified survey map; or provisions for the protection
of any environmentally significant lands within the boundaries of
the proposed certified survey map.
K. Any additional information as requested by the Village Zoning Administrator
or Village Plan Commission.
Where the certified survey map is located within a quarter section,
the corners of which have been relocated, monumented and coordinated,
the certified survey map shall be tied directly to two of the section
or quarter corners so relocated, monumented and coordinated. The exact
grid bearing and distance of such tie shall be determined by field
measurements, and the material and Wisconsin State Plane or Waupaca
County Coordinates of the monument marking the relocated section or
quarter corner to which the plat is tied shall be indicated on the
certified survey map. All distances and bearings shall be referenced
to either the Wisconsin State Plane Coordinate System or the Waupaca
County Coordinate System. Use of the Waupaca County Coordinate System
is encouraged. If the plat provides county monument coordinates, then
these shall be accurate to four decimal places.
The surveyor shall certify on the face of the map that he has
fully complied with all the provisions of this chapter.
A. Dedication of streets and other public areas shall require the owner's
and mortgagee's certificate in substantially the same form as
required by §§ 236.21(2) and 236.34(1)(e), Wis. Stats.,
and the approval of the Village Board.
The certified survey map shall only be recorded with the County
Register of Deeds after the certificates of the Village Zoning Administrator
or Village Plan Commission, the surveyor, and the Village Board are
placed on the face of the map.
