The City of Hometown, Illinois, is for the purpose of conducting municipal elections, including primaries, and for such other purposes as hereafter may be provided by law, divided into five wards, which shall be numbered and comprise the territory as follows:.

The boundaries of the various wards shall be as follows:
The ward map, together with a description of the ward boundaries, identified as "Official Ward Map for the City of Hometown", is hereby adopted as the official City map for aldermanic election purposes as specified in Exhibit A.[1] Wards of the City shall be properly identified by the word "ward" followed by the Arabic numeral corresponding to the ward number assigned by the redistricting ordinance.
A copy of the map showing the area of the boundaries of each ward together with a supplement describing the boundaries using census blocks as determined by the United States Census Bureau of each ward is on file in the City Clerk's office as attached as Exhibit A. In general, the centerline of the identified right-of-way in a redistricting ordinance shall be the boundary line unless otherwise noted.
Territory annexed into the City following the adoption of a redistricting ordinance shall, upon annexation and without further action by the city council be made a part of the adjacent or surrounding ward; provided that whenever two or more wards surround or are contiguous with the annexed territory the City Council shall provide as part of the ordinance annexing the territory what ward or wards the territory shall be made a part of.

Wherever a street is indicated as the boundary of a ward, the center line of such street shall be considered to be the boundary line of such ward; provided, however, that if such street or some part thereof constitutes some portion or all of the distance for which said street is hereinabove designated as the boundary of any ward, then that part of the street which is the boundary of the City shall also be the boundary of such ward; and provided further that if any streets are not dedicated throughout the entire distance for which it is indicated as one of the boundary lines of any ward, then the prolongation of the distance for which it is indicated as one of the boundary lines of any ward, then the prolongation of the center line of such street shall be considered to be such boundary line.
