1. 
Before any building permit is issued by the Township of Hunlock to any contractor or subcontractor, the contractor or subcontractor shall provide to the Township the following information:
A. 
Workers' Compensation policy number.
B. 
Name of Workers' Compensation insurance company.
C. 
Contractor's or subcontractor's federal or state employer identification number.
D. 
Number of employees of the contractor or subcontractor.
2. 
If the contractor or subcontractor signs an affidavit stating that he has no employees and is not required to carry Workers' Compensation insurance, the Township of Hunlock may issue a building permit stating the contractor's or subcontractor's federal or state employer identification number, and a notation that the contractor or subcontractor has no other employees and does not carry Workers' Compensation insurance, and that the contractor or subcontractor is not permitted to employ any individuals to perform work under the building permit.

As proof of insurance, the Township shall require the contractor or subcontractor to supply, either directly or through the contractor's or subcontractor's insurer or agent, a Workers' Compensation Certificate, which includes the effective date of the coverage and the signature of the insured. This certificate must be kept on file with the building permit.

This section shall not affect the issuance of building permits to noncontractors or nonsubcontractors. Property owner applicants are not "contractors and/or subcontractors" within the meaning of this section.
5. 
Nothing herein shall affect the validity of any other ordinances of the Township concerning the issuance of building permits. This Part is intended as an addition to any existing Part concerning the issuance of building permits, and is intended to apply to contractors and subcontractors only.
