Unless the person proposed to fill the position for appointment is expressly mandated under statute, ordinance, or other regulation, any person desiring to be appointed by the Mayor, City Council, City Manager, or other appointing authority to any City commission, citizens advisory committee, City board or City authority shall file a written application with the City Manager on a form to be furnished by the City Manager. The applicant shall tender with the application the following information:
A. Name, complete residence address and residence telephone number.
B. Two previous addresses immediately prior to the present address of the applicant.
D. Current employer, position, address, telephone number, and name of direct supervisor.
E. General personal history as would bear upon the character or reputation of the applicant, including previous public service experience, special skills or trades, or other qualifications unique to the position being applied for.
F. Written proof of any license, certification, or other expertise, if required, pursuant to any other ordinance of the City of Roseville.
G. All criminal convictions, fully disclosing the jurisdiction in which convicted and the offense for which convicted and the circumstances thereof.
H. A waiver and release to allow the City of Roseville Police Department to conduct a criminal history background check, including the use of the Law Enforcement Information Network (LEIN).
