The sketch plan shall show the following data and be submitted prior to the application for conditional approval:
A. 
The name of the proposed subdivision.
B. 
The scale (minimum of one inch equals 100 feet), date, North point, tax lot designation, name and address of the recorded owner.
C. 
Contours with intervals of five feet or less, referred to the National Geodetic Vertical Datum.
D. 
The location of existing property lines, streets, easements, buildings, watercourses and marshes on the site.
E. 
All proposed streets, lots, recharge basins, open space and easements.
F. 
The application fee in a sum as required under a fee schedule established by the Board of Trustees, on file at the Village Clerk's Office and amended by resolution from time to time by the Board of Trustees as need arises.

The following documents shall be submitted for conditional approval:
A. 
A preliminary plat prepared at a scale of not less than 100 feet to the inch showing:
(1) 
The proposed subdivision name and the date, North point and scale, the name and address of the record owner, subdivider and engineer or surveyor and the total acreage and total number of proposed lots.
(2) 
The names of all owners of record of all adjacent property.
(3) 
School or special district boundaries.
(4) 
The location of existing property lines, streets, easements, buildings, watercourses and marshes. The location of rock outcrops, wooded areas, single trees with a diameter of eight inches or more, measured three feet above the base of the trunk, and other significant existing features shall be approximately shown.
(5) 
The location of existing storm sewers, septic tanks, water mains, wells, culverts and drains on the property, with pipe sizes, grades and directions of flow.
(6) 
Contours with intervals of two feet or less, referred to the National Geodetic Vertical Datum; the approximate grading plan showing proposed street gradients and critical elevations at high and low points and at intersections of streets.
(7) 
All proposed streets, lots, easements and public or community area, with approximate dimensions.
(8) 
The approximate location of all proposed waterlines, valves and hydrants and of all sewer lines, indicating connection with existing lines or alternate means of water supply or sewage disposal and treatment.
(9) 
A preliminary storm drainage plan.
(10) 
The names of all proposed streets and other accessways.
B. 
The size of the plan shall be no larger than 30 inches by 42 inches.
C. 
If the application covers only a part of the applicant's entire holding, a map of the entire tract, drawn at a scale of not less than 400 feet to the inch, showing an outline of the platted area with its proposed streets and an indication of the probable future street system in the remaining portion of the tract and the probable future drainage layout.
D. 
A copy of such covenants or deed restrictions as are intended to cover all or any part of the tract.
E. 
An application for conditional approval of a subdivision and a completed environmental assessment form as required by Chapter 66 of this Code.
F. 
The engineering review fee, plus the application fee in a sum as required under a fee schedule established by the Board of Trustees, on file at the Village Clerk's Office and amended by resolution from time to time by the Board of Trustees as need arises.

The following shall be submitted for plat approval:
A. 
The plat shall be drawn in ink on tracing cloth or Mylar in sheet sizes of either 18 inches by 20 inches or 20 inches by 36 inches and at a scale of not less than 100 feet to the inch. When more than one sheet is required, an additional index sheet of the same size shall be filed showing to scale the entire subdivision, with lot and block numbers clearly legible. The plat shall show:
(1) 
The subdivision name, date, scale and North point.
(2) 
The certification of title showing ownership.
(3) 
The names of the owners of adjacent land.
(4) 
Certification by a licensed surveyor as to the accuracy of the survey and plat, as follows:
"This plat has been prepared from a boundary line survey, and all lots shown hereon comply with the laws, regulations and ordinances of the Incorporated Village of North Haven. All monuments shown hereon will be set.
(Signed by licensed surveyor)"
(5) 
Primary control points (whenever possible, including monuments included in the state system of plane coordinates or reference points previously established by public authority) or descriptions and ties to such control points, to which all dimensions, angles, bearing and similar data on the plat shall be referred.
(6) 
Boundaries of the property; building or setback lines if different from those required in the Zoning Ordinance[1] and lines of streets, lots, reservations, easements and area to be dedicated to public use; the lengths and bearing of all straight lines; radiuses, arc lengths and central angles. All lengths shall be in feet and decimals of a foot to the nearest 0.01 of a foot. All angles shall be given to the nearest 10 seconds or closer if deemed necessary by the surveyor. The error of closure shall not exceed 1:10,000.

(7) 
The area of all lots, parks, open space or other parcels for which a tax lot number is assigned, to be stated in square feet.
(8) 
The location, material and size of all permanent monuments (two per street intersection, at all angle points in the perimeter boundary line and one at all changes of direction of streets or dedicated parcels).
(9) 
The proposed street names, block, lot and section numbers in sequence as directed by the Planning Board and the tax lot numbers as assigned by the Suffolk County Department of Real Property.
(10) 
A key map at a scale of 600 feet to the inch showing proposed subdivision streets in relation to the existing road and highway system with a tie to the existing highway intersection.
B. 
Deeds of cession of streets, rights-of-way, easements and any sites for public use and copies of agreements, covenants or other documents showing the manner in which areas to be reserved for the common use of the residents of the subdivision are to be maintained, all certified as to their legal sufficiency by the Village Attorney.
C. 
Cross sections and profiles of all proposed streets showing existing and proposed grades as approved by the Village Engineer. The cross sections shall show pavements and gutters where required.
D. 
If street trees are to be planted by the subdivider, planting plans showing the types and spacing of trees.
E. 
Profiles along the center lines of streets showing existing and proposed elevations. Where a proposed new street intersects an existing street, the elevation of such existing street shall be shown for 100 feet from such intersection. All elevations must be referred to the National Geodetic Vertical Datum. The size, slope and inverts of all drainage pipes and structures and all utilities shall be indicated on the profile. The scale shall either be four feet vertical and 40 feet horizontal to the inch or five feet vertical and 50 feet horizontal to the inch.
F. 
Development plans showing the location, size and invert elevations of existing and proposed storm drains, water mains and utilities. Fire wells or hydrants and proposed and existing buildings shall be shown. Existing and proposed contours shall be shown at a two-foot interval. Street names and lot numbers shall be shown on the plan. The plan shall be prepared on a sheet no larger 24 inches by 36 inches at a scale no larger than one inch equals 60 feet. A title box in the lower right-hand corner shall show the name of the subdivision map, the date, scale and the name of the person preparing the plan. A North arrow shall also be shown. The plan shall be prepared by a licensed land surveyor or professional engineer and shall be signed and sealed by him.
G. 
The engineering review fee and the final plat application fee in a sum as required under a fee schedule established by the Board of Trustees, on file at the Village Clerk's Office and amended by resolution from time to time by the Board of Trustees as need arises.
