[Adopted 3-4-1974]
The following written statement of facts shall be presented to the Board of Trustees before said Board will consider any proposed resolution for the laying out, altering, widening, narrowing, discontinuing or accepting the dedication of a street in the village:
A. 
If the proposal is for the laying out of a street, the general course thereof and a description of the land to be taken.
B. 
If the proposal is for the alteration of a street, its names, the proposed alteration and a description of the land, if any, to be taken.
C. 
If the proposal is for the widening of a street, its name and a description of the land to be taken.
D. 
If the proposal is for the narrowing of a street, its name, its proposed width after such alteration and the manner in which such narrowing is to be effected.
E. 
If the proposal is for the discontinuance of a street, its name and the part proposed to be discontinued.
F. 
If the proposal is for the laying out, alteration or widening of a street, the names and residences of the owners of all land to be taken.
G. 
If the proposal is for the narrowing or discontinuance of a street, the names and residences of the owners of adjoining lands effected.
H. 
If the proposal is for the acceptance of a dedication, the width and length of such street, the name and residence of each owner of land adjoining the same and the improvements to be included in such dedication.

If such proposal shall be requested by other than a member of the Board of Trustees, it shall further set forth the name(s) and address(es) of the proposer(s), and said proposer(s) must sign such statement and deposit a sum as set by resolution of the Board of Trustees from time to time[1] to cover the expenses for publishing and serving notices of any public hearing to consider any resulting resolution.
