incorporated herein are hereby approved and adopted.[1]

The Disclosure Statement, substantially in the form set forth
in Schedule 2, attached hereto and incorporated herein, is hereby
approved and adopted.[1]

The form of the Petition for Appeal and Refund, substantially
as set forth in Schedule 3 attached hereto and incorporated herein,
is hereby approved and adopted.[1]

The governing body hereby determines that administrative appeal
procedures relating to petitions for appeal and refund submitted by
taxpayers in connection with the assessment, determination or refund
of an eligible tax under the Local Taxpayers Bill of Rights shall
be undertaken by the governing body.

The governing body hereby appoints the City Council to review
and decide all appeals filed pursuant to the Local Taxpayers Bill
of Rights. This designation may hereafter be changed by resolution
of the governing body.
The Administrative Appeal Procedures set forth in the rules
and regulations and substantially in the form set forth in Schedule
4, attached hereto and incorporated herein, are hereby approved and
adopted.[1]

This article shall become effective in accordance with the provisions
of law and has been applicable to eligible taxes as of January 1,
1999.
Any provision contained herein which conflicts with procedures
established by the collection agent designated by the City shall be
read as fully consistent with the procedures of the collection agent.
