A. Composition.
1. The Committee shall be composed of five (5) members that are citizens
of the City of Lexington, citizens in the Lexington School District
or employed in the City of Lexington. Each member shall be appointed
by the Mayor with the approval of the City Council for a term of three
(3) years, the original appointments to the Committee to be made for
the following terms:
a. One (1) member appointed for a term of one (1) year.
b. Two (2) members appointed for a term of two (2) years.
c. Two (2) members appointed for a term of three (3) years.
2. Membership will be composed of the following:
a. One (1) person selected by the Lexington City Hospital Board of Trustees.
b. One (1) person selected by the Lafayette Regional Health Center.
c. Three (3) citizens at large who meet the criteria outlined above.
B. All appointments thereafter shall be for a term of three (3) years.
Vacancies shall be filled by the appointing authority for the unexpired
term of any member whose office becomes vacant. The Committee shall
elect a Chairman and a Secretary who will take minutes. Both shall
serve for a term of office of one (1) year. Officers will be members
of the Committee. The Mayor will appoint a City Council member to
serve as the Committee's liaison to the City.

All citizen members of the Health and Wellness Committee shall
serve without compensation.

The City Council may remove any citizen member of the Health
and Wellness Committee for cause, stated in writing.

A. The Committee will take the necessary actions to promote health and
wellness programs in the City of Lexington and to recommend and award
financial grants to applicants based on the guidelines set out in
the 2015 hospital lease. All grants will be reviewed and approved
by the Lexington City Council. The Committee shall be in charge of
handling all correspondence with the citizens, handling all correspondence
with City Council relative to the health and wellness programs, coordinate
with Lafayette Regional Health Center on programs recommended to the
City Council for implementation and prepare estimates for each project
for City Council approval. The monies may be accumulated, i.e., does
not require being expended the year of donation.
B. It is expected that the awarding of the majority [minimum eighty
percent (80%)] of the grant monies will be for larger, long-term projects
that will serve the health, wellness and well-being of the community
for many years into the future.
C. A simple majority [three (3) of five (5) members] is required to
approve grants. Any grant awarded must have a "yes" vote from either
the LRHC representative or the Board of Trustees representative.
D. The City and/or the grant recipient(s) will recognize HCA Midwest
and LRHC with long-term signage and appropriate news releases for
each grant and project submitted by the Committee and approved by
the City Council.
