The office of the registrar of births and deaths is hereby created, and the city secretary is hereby designated to act in such capacity within the corporate limits of the city.

(a) 
The registrar of births and deaths shall complete, correct, arrange in chronological order and index the birth and death records of the city, and shall forward, prior to the 10th day of the following month, to the state bureau of vital statistics, all original certificates of births and deaths filed during the previous months.
(b) 
The registrar of births and deaths shall tabulate the dates shown on the birth and death certificates and shall make such monthly report to the city council as may be required of that body.

The city secretary, upon the payment of a fee as set forth in appendix A to this code, shall issue a certified copy of any record within his keeping, except such as are forbidden by the laws of the state.
