A.
The fee
for administration of single-dose injections per vaccination shall
be $15.
B.
The fee
per additional vaccination for multi-dose vaccines requiring a series
of injections shall be $10.
C.
The fee
per vaccination for a single-dose intranasal or oral vaccine shall
be $10.
D.
The fee
per vaccination for each additional multi-dose intranasal or oral
dose vaccine shall be $8.
