The application for permit shall be filed in
duplicate; accompanying such application there shall be filed duplicate
plans and specifications showing the proposed work, together with
any other information concerning the operation, not contained therein,
as may be required by the Commissioner of Public Works.
Every application for permit shall designate
where the work is to be done, by lot number and street, or as required
by the Commissioner of Public Works, so as to readily and definitely
identify and locate the proposed work.
Every application for permit shall show the
use or occupancy of all parts of the building and such other reasonable
information as may be required by the Commissioner of Public Works.

Every application for permit shall contain a
sworn statement, in writing, giving the name of the registered architect,
licensed professional engineer or the person, when authorized by Articles
145 and 147 of the State Education Law, by whom such applications
were prepared and the full name and residence of the owner of the
premises or building to be erected, altered, moved or demolished or
in which the installation or alteration of building equipment is to
be made.

Every application for a building permit shall
be accompanied by a fee, as required by this section, payable to the
City of Middletown, through the Commissioner of Public Works.
A. Every building permit shall expire if the work authorized
has not been commenced within 12 months after the date of issuance
or has not been completed within 18 months from such date, on construction
costing less than $1,000,000, and has not been completed within 30
months from such date for construction costing in excess of such amount.
If no zoning amendments or other codes or regulations affecting subject
property have been enacted in the interim, the Building Inspector
may authorize, in writing, the extension of either of the above periods
for an additional six months, following which no further work is to
be undertaken without a new building permit.
B. The following schedule of fees payable to the City
shall apply to every building application. $10 per $1,000 up to $500,000.
(Projects over $500,000 to pay an inspection fee of 2%.)

C. The fee for a permit to demolish a building or structure
shall be set at $200.

D. The fee for a permit to move a building or structure
from one location to another shall be set at $200.
