Any person, firm, corporation, or organization who or which
has secured a bingo license under ILCS Ch. 230, Act 25, § 1
et seq., shall also secure a bingo license from the Village.
Applications for a license shall be made in writing to the Village
Clerk. Each application shall state the name of the applicant, the
location to be used, and the state license or identification number
under the Bingo License and Tax Act, together with a copy of the same.
Each application shall contain such additional information as may
be needed for the proper guidance of the Village officials in the
issuing of the license applied for.
Forms for all licenses and applications therefor shall be prepared
and kept on file by the Village Clerk.
The fee for the issuing of a license shall be $5. Each license
shall expire at 12:00 midnight on June 30 following the date of its
issuance.
Each license issued shall bear the signature of the President
and Village Clerk.
