If a utility bill remains unpaid after the disconnect date as described in Section 700.060(C), a continuation/reconnection charge of twenty-five dollars ($25.00) shall be made during normal working hours. After normal working hours, a continuation/reconnection charge of fifty dollars ($50.00) shall be made.

A. 
The monthly rates required and which shall be charged and collected by this City for water and water service furnished by the Water Department of the City of Sullivan shall be as follows:
1. 
The minimum charge per unit shall be a flat fee of four dollars sixty-nine cents ($4.69) for all water customers.
2. 
For all water consumption, a charge of forty-eight and two-tenths cents ($0.482) per one hundred (100) gallons or fraction thereof will be charged.
3. 
The City shall not charge the fire protection district for water used at their firehouses to be used to extinguish fires nor for water used from fire hydrants to extinguish fires.

A permit and construction fee for a Class A or Class B water connection in an amount equivalent to the material and equipment cost plus ten percent (10%) (as determined from time to time by the Water and Sewer Commissioner) shall be paid to the City Collector at the time the application is filed. A fee of fifty dollars ($50.00) shall be required for purchase of a protective enclosure and tracer wire for a replacement or new water service lateral within the public right-of-way to comply with locating requirements of Chapter 319, General Safety Requirements, Section 319.033, RSMo., within the public right-of-way.

A. 
There shall be two (2) types for user fees on connection of sewer permits:
1. 
Type A. A four-inch sewer tap; and
2. 
Type B. A sewer tap in excess of four (4) inches.
A permit and inspection fee of five hundred dollars ($500.00) for a Type A sewer connection or six hundred dollars ($600.00) for a Type B sewer connection shall be paid to the City Collector at the time the application is filed to cover the material and equipment cost required to make said tap. A protective enclosure and tracer wire, purchased from the City of Sullivan Water Department, shall be required for any replacement or new water service lateral for the portion within the public right-of-way to comply with locating requirements of Chapter 319, General Safety Requirements, Section 319.033, RSMo., within the public right-of-way. A fee as set out in Section 160.310 of this Code shall be paid to the City Collector at the time the application is filed for the connection.

The minimum charge per unit per month shall be a flat fee of eleven dollars eighty-one cents ($11.81). In addition, each residential, commercial or industrial contributor shall pay a user charge for operation and maintenance, including replacement, of sixty and three-tenths cents ($0.603) per one hundred (100) gallons of water use during the current month.
B. 
Sewer charges will not be assessed based on water used by the fire protection district at their firehouses to be used to extinguish fires nor for water from fire hydrants used to extinguish fires.
C. 
The City hereby adopts the following fees and charges for reimbursement of costs of setting up and operating the City's Pretreatment Program for users that qualify under Chapter 725 of this Code, which are separate from all other fees, fines, and penalties chargeable by the City:
1. 
Fees for wastewater discharge permit applications including the cost of processing such applications: Seventy-five dollars ($75.00).
2. 
Fees for monitoring, inspection and surveillance procedures including the cost of collection and analyzing a user's discharge and reviewing monitoring reports submitted by users: One thousand five hundred dollars ($1,500.00).
3. 
Fees for reviewing and responding to accidental discharge procedures and construction: Twenty-five dollars ($25.00) per hour per person for staff who work on the accidental discharge resolution.
4. 
Fees for filing appeals: Seventy-five dollars ($75.00) plus any actual costs incurred.
5. 
The City reserves the right to from time to time establish other fees in support of the program.

Application for the use of water and sewer services shall be made to the City Collector at the City Hall by the consumer. Before service is furnished hereafter, said applicant shall be required to pay a water and/or sewer account establishment fee for all water and/or sewer service for which receipt will be given by the City Collector. The establishment fee to be as follows:
Residential fifty dollars: $50.00.
Commercial (less than 35 kW demand): $100.00.
General power (35 kW demand or more): $200.00.
The establishment is based upon the electrical customer account classification when electric service is to be provided by the City. In the absence of electric service by the City, the best classification will be determined by the City based upon the intended use of the service.
Application for the use of electrical service shall be made to the City Collector at the City Hall by the consumer. Before service is furnished hereafter, said applicant shall be required to pay a connection fee for all electrical service for which receipt will be given by the City Collector. The connection fee to be as follows:
Residential fifty dollars: $50.00.

In addition to the above compensation for providing electric service, the City of Sullivan, Missouri shall collect a purchased power adjustment on the sale of each KWh to each customer. For each one cent ($.01) increase or decrease or major fraction thereof, in the City's monthly base purchased power cost of $0.0708 cents per KWh in the first three (3) of four (4) months preceding the billing period, there shall be correspondingly added to or subtracted from each customer's bill an amount equal to one cent ($.01) per KWh times the total KWh billed to the customers on all of the City's metered kilowatt hour rates.

A. 
For the residential rate, commercial rate, and all other non-demand metered rates, the charges in Sections 160.350 and 160.360 of this Code are subject to an annual operations adjustment. Commencing with the first billing date for each consumer after November 30 of each year, there shall be an increase of three percent (3%) to the customer service charge and an increase of three percent (3%) to the difference between the energy charge and the base energy charge of $0.0708 as defined in the purchased power adjustment.
B. 
For the general power service rate and all other demand metered rates, the charges in Section 160.370 of this Code are subject to an annual operations adjustment. Commencing with the first billing date for each customer after November 30 of each year, there shall be an increase of three percent (3%) to the customer service charge, and an increase of three percent (3%) to the difference, if positive, between the first block energy charge per KWh and the base energy charge of $0.0333 per KWh, and an increase of three percent (3%) to the difference, if positive, between the second block energy charge for additional energy and the base energy charge of $0.0333 per KWh.
There is established in the City of Sullivan, Missouri, a charge of thirty dollars ($30.00) for each temporary service installed. This fee is non-refundable.
