A permit for the sale of intoxicating liquor for medicinal purposes
pursuant to § 125.61(1) through (6), Wis. Stats., may be
granted to a registered pharmacist upon action by the Village Board.
A separate application for each premises shall be made to the Clerk-Treasurer
upon forms provided to him/her.

Upon the approval of the application by the Village Board, the Clerk-Treasurer
shall issue a permit to the applicant. A medicinal permit shall be
issued free of charge, and is not subject to § 125.04(11)(a),
Wis. Stats.
Each permit shall be numbered in the order in which issued and shall
specifically state the premises for which issued, the fee paid if
applicable, and the name of the licensee.
License required. No person, firm or corporation shall in any manner,
directly or indirectly, upon any premises, or by any device sell,
exchange, barter, dispose of, give away, or keep for sale any cigarette,
cigarette paper or cigarette wrappers or any substitute therefor without
first obtaining a license as hereinafter provided.
Application for license; fee. Every person, firm or corporation desiring
a license under this section shall file with the Clerk-Treasurer a
written application therefor, stating the name of the person and the
place for which such license is desired. Each license shall be filed
by the Clerk-Treasurer and shall name the licensee and the place wherein
he/she is authorized to conduct such business, and the same shall
not be delivered until the applicant shall pay to the Clerk-Treasurer
a license fee of $5.
Issuance and term of license. Licenses for the sale, exchange, barter,
disposition of, giving away or keeping for sale of cigarette paper
or cigarette wrappers or any substitute therefor shall be issued by
the Clerk-Treasurer. Each license shall be issued on July 1 of each
year, or thereafter whenever applied for, and shall continue in force
from date of issuance until the succeeding June 30, unless sooner
revoked for any violation of this section.
Any person
who knowingly provides materially false information in an application
for a cigarette and tobacco products retailer license under this section
may be required to forfeit not more than $1,000.
