The following fees and charges are established which shall be paid to the Administrative Office of the Board of Health prior to providing the service or report in question:
a. 
On-site subsurface sewage disposal system inspection and report shall be $60.
b. 
Blood chemical screening clinic for adults in connection with the Chronic Disease Control Program of the Board. For 1984, the fee shall be $12 per screening. Thereafter the fee shall be in accordance with the amount charged the Board of Health by the laboratory performing the screening not to exceed $20. The amount so charged, once known, shall be posted in a prominent place in the Montgomery Township Municipal Building reserved for official Township notices.
c. 
An escrow account for review of wastewater treatment facilities such as but not limited to Cycle-Let, spray irrigation, holding lagoons, etc. is hereby established. The amount of the escrow required for the review of the wastewater treatment facility shall depend on the nature and complexity of the design submitted, but in no instance shall such escrow amount required be less than $1,500.
d. 
Seasonal Influenza and Pneumonia Vaccinations.
1. 
The fee for the administration of seasonal influenza and pneumonia vaccinations shall be in accordance with the most recent fee schedule published by the Center for Medicare Services.
2. 
If the vaccine's recipient is enrolled in Medicare, the Township shall receive reimbursement of the fee for the administration of the vaccinations from Medicare instead of from the vaccine's recipient.
3. 
The Health Officer shall develop procedures allowing for a waiver from the fee for the administration of the vaccinations as needed to protect the Public Health or in the event that the payment of the fee would place a financial burden on the vaccine's recipient.

This chapter shall not affect fee schedules which apply to other reports, certificates, permits or inspections as may be set forth in other ordinances or regulations of the Board of Health.

The Registrar of Vital Statistics of the Township of Montgomery, or his/her duly authorized representative, shall be empowered to issue, on behalf of the Township, certified copies of record of all births, deaths, and marriages which occur in the Township.

Certified copies of record for all births, deaths, marriages and civil unions shall be issued at a fee of $20 each.

The fee for completion and filing of a correction to a vital record (birth, marriage, death or civil union) that has already been filed with the State Registrar of Vital Statistics is hereby established as $35.

For copies of computerized information/data of public record on file in the Health Department to zip drive disks, the fee is hereby established at $20 per disk.

For Health Department Inspector collection and transport of water potability samples, the fee is hereby established at $50 per realty improvement.

For all Board of Health license, permit and registration renewals, a late fee of $15 per month will be charged for the submission of such license, permit or registration, or for the submission of any required supporting documentation for such license, permit or registration submission after the specified due date.
