Application for a soil percolation test permit
shall be made with the Town Director of Health for each subsurface
sewage disposal system in writing on forms prescribed by the Town
Director of Health.
A plot plan shall be submitted to the Town Director
of Health for every subsurface sewage disposal system, and every plan
shall include all pertinent information as to basis of design, soil
condition, groundwater and ledge rock elevations, property lines,
building locations, watercourses, groundwater and surface water drains,
nearby wells, water service lines and reserve areas for leaching systems.
No construction of any dwelling, apartment,
boardinghouse, hotel, or commercial facility shall be permitted in
the Town of Haddam unless sewage disposal system plans have been approved
by the local Director of Health or his authorized agent.
An application fee of $25 for each subsurface
sewage disposal system shall accompany the application for a soil
percolation test permit. All plans for subsurface disposal systems
shall be inspected on site, and all percolation tests shall be observed
or performed by the Town Director of Health or his authorized agent.
Payment shall be made by check and payable to the Town of Haddam.
Any person who violates any of the above shall
be fined not more than $100.
The Town Health Director must act upon the application
within two weeks.
