No person shall operate or permit to be operated
any taxicab upon the streets of the city by any person not the holder
of a valid taxicab driver permit issued under this article.
Application for a taxicab driver permit shall
be made upon forms furnished by the Police Chief or his designee and
shall be sworn to and filed with the Police Department. The application
shall set forth:
A. The name and address of the applicant and the date
and place of the applicant's birth.
B. The number of his state driver's license and a statement
as to whether or not such license has ever been suspended or revoked
and the reasons therefor.
C. The name and address of his employer, if any.

All driver permits shall be issued for the calendar
year and shall expire on December 31 of each year. A permit fee shall
be paid to the Police Department upon the filing of each application
for each year in the amount of $40.
