Any and all requests for the dedication of any Village property,
including buildings, streets, trees, benches and/or other Village
property, shall be made in writing and filed with the Village Manager's
office. Such written request must include at least the following information
and supporting documentation:
A. The name of the person to be honored, hereinafter "honoree";
B. Written authorization to make such request from a family member of
the honoree such as the most immediate living family member (i.e.,
spouse, parent, child, blood relative, next of kin) or, in the absence
of any known living relations, attestation to the lack of any kin
and a detailed description of the relationship of the person authorizing
the request on the behalf of the honoree;
C. In the event the honoree is a member of a community organization
or group, a written authorization from the president or executive
of such group or organization must be provided with the dedication
request.
D. The nature, extent and description of the Village property requested
to be dedicated, and a detailed description of the type of remembrance
requested;
E. Acknowledgment and agreement by the person(s) requesting the dedication
that they will be responsible for the costs of replacement and installation
of the remembrance if necessary; and
F. The estimated amount they will contribute to the dedication of the
property.
Before a request for the dedication of a Village asset shall
be considered by the Village Council, the following criteria must
be met by the honoree(s) who is to be considered for such dedication:
A. Village employee. If the person(s) in whose name such dedication
is requested is presently or formerly a Village employee, such person
must have been employed for a minimum of 10 consecutive years and
been in good standing at all times.
B. Volunteers. If the person in whose name the dedication is requested
was a volunteer or noncompensated member of any Village board, committee,
or other Village-sponsored organization, said person shall have served
at least two terms in said position.
C. Others. Person(s) who have volunteered time or other valuable consideration
to the Village whose name(s) is(are) requested to be honored by the
dedication of Village property must have been or presently is(are)
a resident of the Village and contributed to the Village in such a
manner as to distinguish his or her contribution as an exceptional
service to the Village and its residents.
In the case of a dedication of a plaque, the standard specification
shall be as follows:
A. Height. The top of the plaque shall be no more than 12 inches from
the ground and shall be at an approximate thirty-two-and-one-half-degree
angle of the ground.
B. Material: cast bronze or brass.
C. Dimensions: eight inches by four inches.
D. Dedication. Only the following wording may be used:
(3) Title or relationship with the Village;
(4) Date of birth, employ or service; and
In the case of the dedication of any street sign, the standard
specification shall be that the sign color shall be brown with white
lettering and six inches by 30 inches in size.
In the case of the dedication of any tree, the standard specification
shall be marked by a concrete block, 12 inches by 11 inches in size,
set in the ground with a bronze or brass plaque measuring 10 inches
by nine inches placed thereon.
In the case of the dedication of any bench, the standard specification
of any bronze or brass plaque set thereon shall be three inches by
four inches.
In the case of the dedication of any park or land area, the
standard suggested type of material shall be concrete or stone or
other suitable material as may be approved by the Village Council.
In the case of a stone or concrete marker, the specifications shall
be 12 inches by 11 inches in size set on the ground with a bronze
or brass plaque measuring 10 inches by nine inches.
In the case of the dedication of any building, the standard
specification of any markers to be placed on the building shall be
a bronze or brass plaque measuring 18 inches by 24 inches.
