The Town of Georgetown and the Workmen's Compensation Law of Delaware require that the standard "Employer's Report of Occupational Injury or Disease" form be completed and filed with the Town Manager within two working days after the injury occurs. Questions on the form must be answered as completely as possible by the employee's immediate supervisor.
The guidelines listed below will be followed in the event of an on-the-job injury:
A.
Emergency first-aid treatment should be administered, whenever practical, and the injured employee should be transported to Beebe Medical Center or Milford Memorial Hospital Emergency Room. The method of transportation, whether by the nearest available vehicle or by ambulance, shall be determined by the extent of injury and by the person handling the situation.
B.
The injured employee, or any other designated person, should notify the injured employee's supervisor as soon as practical after the injury occurs.
C.
After the injured employee has received medical treatment he or she shall fill out the "Employee's Report" form. The supervisor then shall fill out the "Supervisor's Report" form. The injured employee will then fill out the "Authorization For Medical and Records" form.
D.
After the above three forms have been filled out the Supervisor and the Town Manager will complete the "Employer's Report of Occupational Injury or Disease" form.
E.
The completed forms will be delivered to the Town Manager for processing.
F.
Prior to returning to work from a "lost-time injury" the employee must submit, to his/her supervisor, a signed physician's statement authorizing that employee's fitness to return to full work status.
Town employees are covered by the Delaware Worker's Compensation Act and are required to report all injuries or possible injuries arising out of and in the course of employment to their supervisor by the end of their normal daily work schedule. Worker's compensation benefits will be paid in accordance with the current worker's compensation policy and applicable state law.
