A. 
When filling out the application, answer all questions completely and truthfully and sign it under oath before the Clerk or a notary.
B. 
A background check will be done on every applicant.
C. 
You must be at least 18 years of age.
D. 
A municipality may not issue an operator's license unless the applicant has completed a responsible beverage server training course. Applicants are exempted from the training course requirement if:
(1) 
They are renewing an existing operator's license.
(2) 
Have completed the training course within the last two years.
(3) 
Have held a retail license, manager's or operator's license anywhere in the state within the last two years (§ 125.17[6], Wis. Stats.).
E. 
Subject to Wisconsin's Fair Employment Law, Ch. 111, Wis. Stats., the applicant may not have been convicted of a felony or be an "habitual law offender."
F. 
The Arbor Vitae Town Board has set a policy not to approve an application for anyone who has two or more OWIs within a five-year time period.
G. 
The fee is $25 and the license is a one-year license that expires on June 30th.

A provisional operator's license may be issued by the Town Clerk in accordance with § 125.17(5), Wis. Stats., only to a person who has applied for an operator's license. A provisional license may not be issued to any person who has been denied a license by the Town Board. A provisional license expires 60 days after its issuance or when a regular operator's license is issued to the holder, whichever is sooner. The fee for a provisional operator's license shall be $15.
All operator and provisional operator licenses in the Town of Arbor Vitae will need board approval before issuing effective the General Meeting of the Town Board on April 13, 1994. Applications will be considered on the second and fourth Wednesday of each month. A provisional license will only be issued upon proof of registration in the "Responsible Beverage" class offered at Nicolet College.

No initial or renewal alcohol beverage license shall be granted for any premises for which taxes, assessments or other claims of the Town of Arbor Vitae are delinquent and unpaid.
No initial or renewal alcohol license shall be granted to any person if any or all of the following prevail:
A. 
Delinquent in payment of taxes assessments or other claims owed to the Town of Arbor Vitae.
B. 
Delinquent in payment of forfeiture resulting from a violation of any ordinances of the Town of Arbor Vitae.
C. 
Delinquent in payment to the state of any state taxes or assessments owed.
When applications for licenses or permits pertaining to alcohol beverages are submitted to the Town Office, a check to cover the fee must accompany the applications. A date to acquire the license or permits will be pre-established at which time you will pick up licenses or license. Two weeks prior to the date of pickup, your check will be submitted to the Town depository.
