It shall be the duty of every borough employee
to immediately report to his department head the happening of an accident
coming to his attention which occurred to a member of the public in
a public building, a public park, a public recreation area or on a
sidewalk adjoining the aforesaid public places.
It shall also be the duty of every borough employee
to report to his department head of the occurrence of any accident
involving borough equipment.
The report shall give full details of the accident,
including the date, time, location, weather condition, nature of the
injury or damage, the name and address, if known, of the injured person
and a full description of any vehicle involved.
Accidents coming to the attention of the department
head, not known to any of his employees, shall be reported by him
in the same manner and form.
The Borough Clerk, or the employee assigned
thereto, shall promptly notify the public liability insurance company
for the borough of the reported accident in such manner and form as
may be required by said insurance company.
All employees and department heads shall promptly
cooperate with the insurance company of the borough in the investigation
and disposition of accidents reported as herein required.
Every borough employee incurring an accident
or illness requiring absence from duty shall promptly notify his department
head of the same, giving the nature thereof, name and address of the
physician in attendance, the name and address of any hospital where
confinement is required and the probable length of time for such absence.
