The attached notice of tort claim form shall
be and hereby is adopted as the official notice of tort claim form
for the Town of West New York.[1]

claim form are on file in the office of the Town Clerk.
All persons making claims against the Town of
West New York, its agencies or subsidiaries, pursuant to the New Jersey
Tort Claims Act, N.J.S.A. 59:8-1 et seq., be required to complete
the form herein adopted as a condition of compliance with the notice
requirements of the New Jersey Tort Claims Act.
