The Board of Supervisors shall and hereby is empowered to establish a form document entitled "Civil Enforcement Notice," which may be amended from time to time in the discretion of the Board of Supervisors. Such Civil Enforcement Notice shall contain such information as deemed appropriate by the Board of Supervisors, including, as a minimum, the following information:
A. 
Name, address, and phone number of the violator;
B. 
Location, date, and time of the violation;
C. 
Citation to the ordinance violated and a general description of the nature of the violation;
D. 
Amount of penalty imposed and time for payment;
E. 
Name and address of the entity to whom the penalty is to be paid;
F. 
Signature and title of the officer issuing the notice.
Service of the Civil Enforcement Notice shall be effected by the Enforcement Officer hand delivering a copy of the notice to the violator and/or by mailing a copy of the notice to the violator by first-class mail, postage prepaid and by certified mail, return receipt requested.
