Applications for amusement licenses are made in writing to the
Village Clerk and shall include the following:
A. Applicant information. The name, address and occupation of the applicant.
B. Purpose of application. The license shall specify the object of the
exhibit and the time for which it has been granted.
C. Criminal record of applicant. Whether the applicant has ever been
convicted of a crime.
D. Person in charge. The name and address of the person in charge of
the place of amusement to be held chargeable for any offenses which
occur.
E. Games to be operated. A complete description of each game to be operated,
including manufacturer's name and address, and the serial number of
each device.
F. Additional information. Additional information as shall be set by
resolution of the Village Board.
Anyone convicted of operating an unlicensed carnival or circus
shall be fined up to $50, imprisoned for up to 30 days, or both.
Any amusement device operated in violation of this chapter may
be seized and, upon conviction of its operator for violating this
chapter, may be destroyed.
