A water and sewage assistance program is hereby created to provide financial
assistance with water and sewage bills to individuals and families residing
in the Borough of Monaca.
Water and sewage customers will be asked to voluntarily contribute an
additional $1 on their quarterly water and quarterly sewage bills. Contributions
received will be accounted for in accordance with generally accepted accounting
procedures and placed by the Borough of Monaca in an account titled "Water
and Sewage Assistance Program." Said account shall be with one of the depositories
authorized and designated by the Borough of Monaca to hold and maintain funds
of the Borough of Monaca. The depository shall provide insurance through either
the F.D.I.C. or F.S.L.I.C. Contributions received shall be used solely for
providing financial assistance to qualified applicants requesting financial
assistance with their water and/or sewage bills and for no other purpose.
Any person residing in the Borough of Monaca who receives water and
sewage services supplied by said Borough shall be eligible to make application
for financial assistance from the Water and Sewage Assistance Program on a
form prescribed by the Board of Review, subject to the approval, by resolution,
of the Council of the Borough of Monaca. The application shall require the
applicant, inter alia, to state the applicant's and/or applicant family's
income and expenses, provide reasonable documented proof of said income and
expenses, sign the application and verify that the statements made and information
provided are true and correct subject to the penalties provided in 18 Pa.C.S.A.
§ 4904 relating to unsworn falsifications made to authorities.
A Board of Review is hereby created, the membership of which shall consist
of the President of the Borough Council, the Borough Chairman of the Council
Water Committee, Manager/Secretary and two residents of the Borough of Monaca
who shall be appointed by and serve at the pleasure of the Borough Council.
The President of the Borough Council shall serve as the Chairman of
the Board of Review. The Water Committee Chairman shall serve as the Vice
Chairman of the Board of Review. For the taking of any action, a quorum shall
not be less than a majority of all the members of the Board.
The Board of Review shall review all applications submitted, determine
the applicant's eligibility for assistance and thereafter grant or deny
the application.
A.
Guidelines shall be established and used by the Board
of Review and the Council of the Borough of Monaca in determining eligibility
and the granting of assistance. The guidelines shall be established according
to the standards adopted by the United States Department of Housing and Urban
Development for low- and moderate-income individuals.

The eligibility guidelines may be amended from time to
time by resolution of the Council of the Borough of Monaca.
C.
Each applicant for assistance shall be provided with
a list of the eligibility guidelines upon the making of an application.
A.
Upon the granting of an application for assistance, payment
will be made from the Water and Sewage Assistance Program account to the order
of the Borough of Monaca, and the applicant's water or sewage bill will
be credited accordingly.
B.
The amount of assistance to be granted each applicant
shall be determined by the Board of Review.
C.
The Board of Review shall prepare guidelines, subject
to the approval of the Council of the Borough of Monaca by resolution, to
be used in determining the amount of assistance to be given an applicant.
Every person's eligibility to receive financial assistance from
the Water and Sewage Assistance Program shall be determined on the application
filed. Payment on behalf of individuals whose applications have been granted
shall be made on a one-time basis.
The Board of Review shall present an accounting of the receipts and
expenditures of the Water and Sewage Assistance Program account at the regular
monthly meeting of the Council of the Borough of Monaca.
