No person shall move or cause to be moved, from any place within
the City to another place therein, or place beyond the limits thereof,
any household goods in bulk without securing from the City Clerk an
Information Form for such moving or transportation within five days
from the date of moving or transportation.

Any person desiring Information Forms for the removal or transportation
of household goods shall make application to the City Clerk setting
forth his name and address and also setting forth the names of the
owners or reputed owners of the property to be moved, the address
and location from which the property is to be moved and the new address
or location of the place to which the property is to be removed.

The City Clerk shall issue the Information Form setting forth
the name of the holder of the Information Form as well as the name
of the owner of the goods to be moved, the address from which the
goods are to be moved and the address to which the goods are to be
delivered. This information shall be signed by the City Clerk.

Any person violating any of the provisions of this article shall
be fined not more than $300 and/or be imprisoned for not more than
90 days.
