There shall be appointed annually by the Division of Health a suitable person to be Registrar of Vital Statistics in and for the City of Perth Amboy, in the County of Middlesex, who shall serve until the first day of January next succeeding his appointment or until his successor is appointed and qualified.
In addition to the duties prescribed to be performed by such officer by the laws of the state, the Registrar of Vital Statistics shall perform such other duties as may from time to time be required by the Division of Health. He shall, in addition to transmitting to the State Bureau of Vital Statistics all certificates of marriage, birth and death received by him, report the same to the Division of Health at least once in each month.
The returns of all marriages, births and deaths required by law or by any ordinance or regulation of the Division of Health to be made by physicians, midwives, nurses, clergymen, Judges, undertakers and other persons professionally officiating at such marriages, births or deaths shall be made to the Registrar of Vital Statistics, and each failure to make return or report of such marriages, births or deaths within the time specified by law shall constitute a separate and distinct offense.
Every person having authority to solemnize marriages shall transmit to the Registrar of Vital Statistics of this city a certificate of every marriage solemnized before him within five (5) days thereafter, and said certificates shall be made out on blank forms furnished by the state for the purpose and shall include all facts required by said forms.
It shall be the duty of the physician or midwife present at the birth of every child born in this city, and in case there is no physician or midwife present, it shall be the duty of the parent or witness present at said birth, to report in writing to the Registrar of Vital Statistics of this city all particulars concerning said birth and called for on the blank forms furnished by the state for that purpose, and said report shall be made within five (5) days after the date of said birth.
In case of any person dying within this city, it shall be the duty of the physician who may have attended during the last illness to furnish to the undertaker or any member of the family a certificate of death, or in case there was no attending physician, the City Physician may be required by the Division of Health to view the body and furnish said certificate, which certificate shall be made out on and shall comprise all the facts stated in the blank forms furnished for that purpose by the state.
A. 
Every midwife and undertaker shall annually register his or her name and address, together with his or her license number, with the local Registrar of the district in which he or she resides.
B. 
Within thirty (30) days after the close of each calendar year, the local Registrar shall make and return to the State Registrar a list of all midwives and undertakers who have been registered in his district during the preceding year.

The fee for residents of the city seventy (70) years or older shall be waived for obtaining a vital record provided that the following requirements are met:
(a)
The customer must be residing in the City of Perth Amboy.
(b)
The customer must be seventy (70) years or older, and show proof of age.
(c)
The customer must be present at the moment of the request.
(d)
The certificate being requested must be his/her own birth/marriage/civil union certificate.
(e)
The customer is entitled to one certificate of each kind per year.
(f)
The customer must comply with all the state mandated requirements for the issuance of birth/marriage certificates.
(g)
The customer may be entitled to obtain his/her spouse's death certificate, provided the requestor is seventy (70) years or older at the time of the request, and show proof he/she is the widow/er.

The Registrar of Vital Statistics shall keep a true and correct account of all fees received by his/her office, and turns the same over to the City's Finance Department on a daily basis or within forty-eight (48) hours after closing of every business day.

The City Registrar shall furnish a certification or certified copy of a birth, marriage, civil union, death or fetal death to the following government agencies without any expense to the government agency while conducting its official/administrative duties:
